The CARE program (Communication, Awareness, Resources, Empowerment) is a FREE one-year diabetes management program that is culturally tailored for Black dialysis patients and includes:
- Patient-centered diabetes management workshops tailored for you
- Vouchers for FREE rides to and from dialysis appointments
- FREE bag of groceries every month
- Opportunities to meet others in your community for social support
The CARE program has been developed in consultation with a Registered Nurse, Registered Dietitian, Certified Diabetes Care and Education Specialist, and Board-Certified Specialist in Renal Nutrition.

The CARE program will be held the 4th Monday and Tuesday of each month in 2023. You may choose if Monday or Tuesday works best for you.
